The Community Cancer Support Service is a two year pilot project, which started recently, and is part of a larger project commissioned by the East of England Strategic Clinical Network, exploring the best ways to offer community support to people with cancer.
This pilot supports people, who have colorectal or breast cancer with secondary disease, and who are registered with a GP practice in Cambridge City, South Cambridgeshire or East Cambridgeshire.
The Cambridgeshire Community Services NHS Trust Community Cancer Support Team, working in partnership with Addenbrooke’s Hospital, is based on the Brookfields Hospital site in Cambridge, will work in collaboration across the community with general practice, palliative care and acute trusts to provide post treatment care.
Kay Hardwick, Team Leader, said: “The service is be delivered by Specialist Cancer Nurses and Support Workers who will provide one-to-one support to patients with long term cancer needs.
“One of the main aims of our specialist team will be to help patients manage the challenges of living with cancer and to support them in managing their condition. We will be working closely with the Oncology Team at Addenbrooke’s Service, Specialist Services and GPs to provide follow-up support at home and help deal with any physical, emotional, social, spiritual and practical concerns that patients, their family or carer may face”
This support and advice will be available any time during an illness, by telephone support, or through home visits or an appointment at a GP surgery. Patients will be referred to the team via the Oncology Team, but as the project progresses the team plan to take referrals from GPs and other healthcare professionals.
The team will be working remotely and will be able to provide follow up support through home visits, consultations at GP surgeries and telephone support. The service will operate seven days a week, Monday - Friday 9am – 8pm and at Weekends and Bank Holidays 9am – 5pm.
